With age the vitreous becomes more liquefied (like rupny jello). As this procedes the vitreous pulls away from the retina - the 'living film" lining the back of the eye. This is known as a vitreous detachment and physically stimulates the retina causing the patient to see lightning streaks in their peripheral vision. As well, a sudden onset of floaters is noticed. Floaters are basically debris at the back of the eye and cast a shadow on the retina. They may appear as translucent threads or insects, and move when the eye moves. This phenomena is more easily observed against a blank background such as a white wall and can be quite annoying. Vitreous detachment is rarely tered until age 45, and occurs more frequently in women. The frequency of this condition increases with age; in younger patients they are likely caused by trauma to the eye or inflammation. Lightning streaks are reported in 25-50% of cases, but this is also a symptom associated with retinal tears, a potentially serious condition. In fact, retinal breaks occur in 15-30% of vitreous detachments caused by the vitreous tugging on the retina as it separates from it. A shower of small floaters or black dots is more characteristic of a retinal tear.
