Tree Lighting Ceremony The Annual Christmas Tree Lighting Ceremony at Georgetown Hospital will take place on Tuesday December 4th in the Hospital Courtyard. This free family-centred event features carol singing, the tree lighting ceremony, refreshments, entertainment and a special visitor from the North Pole. Free parking will be available. Halton Healthcares Home at Last Program Halton Healthcare Services (HHS) new Home at Last Program allows patients to safely leave the hospital and go to the comfort and security of their own homes promptly upon discharge. Funded by the Ministry, the Program provides a companion from Links2Care, a community-based service provider, to come to the hospital and accompany patients home by taxi. On the way home, the companion can help get medication, fill prescriptions and arrange for delivery of groceries. This is a joint pilot project recently launched by the Ministry of Health & Long-Term Care (MOHLTC) in conjunction with Links2Care and Halton Healthcare Services, explains Cathy Raiskums, Manager of Social Work and Discharge Planning at HHS. It is designed to enhance patient flow through our hospitals by facilitating the discharge of patients who either live alone or cannot arrange for someone to take them home until late day or evening. This Ministry initiative partners HHS with a community based organization like Links2Care to enhance continuity of care, notes Denise Hardenne, Vice- President of Programs and Quality Development at HHS. This is a great program that provides an integrated community response and facilitates timely discharge for patients, especially seniors, transitioning from the acute care setting of the hospital back to their home in the community. Christmas Ball tickets now available The Annual Hospital Christmas Ball will take place on Saturday December 1st at North Halton Golf & Country Club. Tickets are $150 per person and are available through Shelley Phipps at 905-873-2252.
