THURSDAY, MAY 17, 2012 THE NEW TANNER 15 New Program to Improve Care for Vulnerable Seniors Peel Halton Dufferin Acquired Brain Injury Services and Extendicare Halton Hills Partnership Provides Alternative Care Options Mississauga Halton Local Health Integration Network (MH LHIN) is pleased to announce the creation of a new program in a long-term care home that will improve care for vulnerable clients. The Extendicare Integrated Partnership will allow Peel Halton Dufferin Acquired Brain Injury Services (PHDABIS) to provide additional supports to aging individuals affected by acquired brain injury (ABI) and: Have medical and physical care needs too complex for a community setting Are older adults who are in Alternate Level of Care (ALC) beds or at home with burdened caregivers and who are in need of long term care - but who previously would not have met the criteria for admission Are currently in long-term care but are experiencing placement challenges due to cognitive/behavioural issues resulting from ABI. This "right care, right support" partnership between PHDABIS and Extendicare will ensure that there is enhanced capacity in the region so that residents of the LHIN in belonging to this wonderful organization. This partnership will provide the long-term care setting with increased resources to care for clients with behaviour issues resulting from ABI thereby reducing unnecessary hospital emergency admissions. This integrated partnership between PHDABIS and Extendicare will result in: A dynamic "needs centred " behavioural support environment, maximizing the efficiencies and synergies; S h a r i n g a n d dissemination of skills, increasing the ability of staff to support cognitive behavioural challenge in a sustained way; Creation of expertise and excellence in the support of ABI and other neurocognitive challenges; Improved flow from hospital for individuals requiring this support for successful placement; Increased availability of treatment beds for younger adults at PHDABIS facilities; Reduction of longterm care discharges to hospital in the face of unmanageable behaviours. PHDABIS provides a comprehensive range of programs and services for acquired brain injury clients, and their families and caregivers, in Mississauga Halton LHIN and beyond. That continuum of programming includes: adult day services, community outreach case management, behavioural consultation, psychological counselling and neuropsychiatric consultation, acquired brain injury education for individuals and families and assisted living for individuals with mild, moderate and severe cognitive and behavioural challenges. The Mississauga Halton LHIN believes that new programs such as this are needed to fill a serious gap in the management of cognitive behavioural challenges in long-term care. This integrated partnership is viewed as a perfect fit since PHDABIS has experience caring for people with ABI and Extendicare Halton Hills provides long-term care. The new resources will provide for staffing with specific expertise serving individuals with physical, cognitive and behavioural response issues while also meeting their medical needs. The LHIN has provided over $240,000 on an annual basis for the effective management of behavioural challenges related to ABI in longterm care homes that will assist in decreased pressure on hospital admissions. DON'T MIND ME: A bear similar to the one pictured above was spotted recently in various locations in the Milton area. - file photo The Halton Regional Police Service has received several bear sighting reports over the past two days in Milton. On Saturday, May 12, 2012, a citizen reported observing a bear in the area of Guelph Line and Highway 401 in the Town of Milton. On Sunday, May 13, 2012, a local farmer reported observing a bear on his property in the area of Sixth Line and Lower Base Road in the Town of Milton. Members of the police service attended and observed a bear in the distance. The bear Officers encounter black bear in Milton area subsequently came towards the police officers and stood up on its back legs about fifty meters away from the officers. The bear then ran off into the bush area. The bear is described as a large male black bear weighing approximately 400 pounds. There is no indication the bear has caused any harm to any human or cattle at this time. Bears can travel great distances and subsequently the exact location of the bear cannot be determined. The Ministry of Natural Resources has been contacted regarding the sighting and is working in partnership with police. The Ministry has a telephone inquiry line to report bear problems namely the "Bear Information/Reporting Line" at 1-866-514-2327 and further safety tips can be located on their website at www.mnr.gov.on.ca. In a non-emergency situation the police service can be contacted at 905 878-5511 to provide any updates on bear sightings. Branch no. 197 Legion news By Sharon Graham P.R.O. This coming Friday we have entertainment on the patio, or if it is raining it will be in the games room. Bad Weather will be here to entertain all of us. Please remember everyone is welcome. We have a BBQ as well on Friday evening so come early and have a bite to eat followed by the entertainment. The dinner will start at 6 p.m., and at 7 p.m., Bad Weather will perform. The results of our elections will be in next week's paper as it was too late for this week. I just want to personally congratulate all of the executives some new some old.
