The Ministry of Health and Long-Term Care and the Mississauga Halton Local Health Integration Network (MH LHIN) recently announced this centre is one of 14 DRCCs being established across the province to implement Ontarios Diabetes Strategy a strategy to prevent and manage dia- betes through early assess- ment and best clinical practices. These regional centres will organize and co-ordi- nate existing regional dia- betes programs, identify local population needs and address gaps in services, according to a release from HHS. They will build on exist- ing local resources and expertise to ensure dia- betes services work togeth- er to increase access for people across the continu- um of care, whether they HHS named lead for diabetes regional centre need preventative, basic or complex diabetes care. With more than 12 years experience of delivering a com- prehensive and well co-ordinat- ed diabetes education program it seemed like a natural fit when HHS was selected to serve as the lead organization for the DRCC for the communities of Halton and Mississauga. Halton Healthcare Services, as the newly appoint- ed Lead for the Diabetes Regional Coordination Centre for the MH LHIN will add their proven experience and expert- ise, in the delivery of compre- hensive well managed diabetes care strategy, to the collabora- tions and services currently in place to improve patient care in our LHIN, said Bill MacLeod, CEO of the Mississauga Halton LHIN. We were delighted to have been chosen to establish the Regional Coordinating Centre for the Ontario Diabetes Strategy for the Mississauga Halton Local Health Integration Network, said Denise Hardenne, HHS senior vice-president. The DRCC will reach out to family doctors, nurse practi- tioners and all other health professionals with new tools and resources that support bet- ter patient screening, shared patient management as well as self-management. It will also help expand the availability of specialized serv- ices for those with more com- plex conditions. Our ultimate goal is to pro- vide individuals living with, or at risk of developing diabetes in Halton and Mississauga better access to information, and serv- ices. We want to make sure no one falls through the cracks that every individual will easily find the highest quality of care possible no matter where they enter the healthcare system, said Patricia Knapp, regional director, Mississauga Halton DRCC. This area already has some very effective and well recognized services in place. We will build on these part- nerships and together develop a strong, well connected care system. The DRCC will play a key role in providing physicians with the most current practices and evidence-based care so they can help their patients take a stronger role in self-managing their diabetes, said Dr. Gail Macpherson, Oakville family physician and Halton Diabetes Program chair. With the DRCC as our driving force and our direct link to the min- istry, we will have even stronger sup- port and advocacy, clearer direction, and better coordination, said Stacey Horodezny, Team Leader, Diabetes Management Centre at the Trillium Health Centre. This will allow us to streamline our efforts and take our services to an even higher level. We are very excited about the new DRCC role.
