SECTION A Wednesday Januarys 19H8 Privileges shouldnt be reinstated hospital lawyer Con from Page Money also played a factor in said Mr The tact that Ford performed more midforceps deliveries when there was a increase in OHtP allowance for midforceps is an in dlcation that Ford a bad tice la continually related to his ability to earn more money said Mr Liswood He said the Board should also have a significant problem with his credibility Reading from hearing of the second session of the hearing Mr recalled a patient which Ford saw just before he left on vacation Ford had testified that because he was on holidays he had very little to do with her management prior to her delivery on Oct Under strict questioning Ford admitted he had seen the woman right up until Oct 11 That not misrepresentation Guidelines and medical staff rules wouldnt work he says That a lie said Mr Liswood of Fords statement The deliberate and embellished He should discredit in totality the evidence from Dr Ford We caught him on this one said Mr Mr Liswood turned his attention to two sets of twins and one stillbirth which be said died as a result of Dr Ford actions In each of these three cases five babies in each case the hospital records show there was a heartbeat viable fetal lite not long before they died Mr said Recalling the evidence given by Mr claimed his witness openly threw his hands up and said the problem here is the man has to go back to school Turning to the evidence offered on the high number of midforceps deliveries performed by Dr Ford Mr Liswood said there was no ex of Dr Ford stopping mid forceps delivery and performing a Caesarian section He also pointed to a case In which a woman was discharged four times with premature ruptured mem Dr Ford said the woman had a social situation which required her at home Dr Ford had no pro blem bringing his entourage down here We dfon hear from this woman said Mr He was referring to about nurses who at ended the hearings daily He rebuffed any suggestion that Ford be given a chance to earn his privileges at the hospital through retraining or setting guidelines Guidelines don work with Ford Medical staff rules don work Matters brought to his attention don work What the point of talking to Dr Ford he said Revoking only his privileges to deliver babies with the use of mid forceps procedure would be Impossi to police said Mr Liswood Executive Director Mark said it would be extremely difficult to police Dr Ford and the only other gynecologist and obstetrician Dr Valerie Kumar would likely refuse the task of attempting to monitor Dr Ford He also defended the hospital a tions In revoking Dr Ford privileges He said Mr MacKenxie failed to differentiate between notice of con cern which he said was given to Dr Ford and notice of steps to revoke privileges which Mr MacKeiule said was never given Mr Liswood challenged the Board to look back on all the decisions made since its Inception in 1973 and find a case where the hospital has come before the Board with such a of contact between the hospital and the doctor under In vestigation Mr attempted to discredit the hospital case by ques tioning the chart review process as a means of Investigation There are many things that you cannot tell about a doctor and his care from charts alone He reminded the Board of evidence given at the beginning of the hearings In October and asked them to keep It in mind when coming to their decision In that evidence several respected doctors In cluding Leonard Landry who sat on the Medical Advisory Committee and knew the details of the cases Ford supported him and practice Those doctors didn t come here totellyoutles He Ford has qualities re Most of the nurses at the hospital say Dr Ford la conscientious ac cessible caring decisive and treats you with respect said Mr MacKenxie He called Ford a credit to the Margaret who said Ford acted above the col of duty in del her baby He asked the Board not to weigh heavily the case In which Dr Ford settled out of court In a malpractice suit for because most of that payment was for lu fees He said altered his prac tice once he was given the criticisms of his work and he Is willing to train to update his knowledge He chastized the Hospital Board for not giving Ford a fair op portunity to be heard before privileges were revoked Mr MscKemle said the Board should remedy the injustice done to Dr Ford Asked by Board Chairman Thomas Wood about their duty to protect the public Interest Mr MacKenxie replied We have to weigh that You can weigh It by restoring restricted privileges MD testifies losing confidence in Ford By BRIAN MacLEOD Herald Staff Dr Donald said he altered his referral practices to Ford after he became concerned about his care of patients In The last three years I rarely sent to Ford for care cause I Just t have con he was giving them the quality of care they should be saldDr Trant He was testifying before the On tario Hospital Appeals Board look Ing into the revoked privileges of gynecologist and Dr Jack Ford He referred to a muchdiscussed case in I960 in which twins died shortly after being born Dr Ford had detected from the woman previous pregnancy that the woman should have a Shorodkar stitch in serted for her next pregnancy because he cervix was not of holding the baby to term He never inserted the stitch Dr Trant thought the woman should have been referred to a tre with better facilities than Georgetown Two years later when Trant was a member or the Medical Ad visory Committee he heard about another case of twins who died at birth The woman was admitted to the hospital with bleeding at weeks the placenta which supplies nourishment for the fetus was separating from the uterus said Dr The patient wasn t treated lively he said The woman delivered twins without a doctor in attendance really didn feel the Judgement was belter he said of the two cases of twins In another case Dr called in Dr Ford to help with a delivery in which the baby was horizon tally in the womb The baby had to be rotated before it could be delivered hesald Dr Ford delivered the baby I was rather surprised to the forceps had been positioned across the face The baby suffered no permanent damagefrom redelivery In another cose In Dr Trant assisted Dr Ford in abdominal surgery In which both the woman a ureters the duct which carries urine from the kidney to the blad der were cut Dr Ford ureters were repaired Immediately Dr Ford earlier testified the woman had a large pelvic moss The mass was huge to say the least The anatomy was totally distorted he said Ford said norma practice is Identify he ureters before the opera Hon so they won be cut He said he sight here no doubt that Inserting catheters was the best way to go about it said Dr Ford in earlier testimony Asked by hospital lawyer Joshua If Dr Trant suggested us ing catheters during the operation Dr Ford replied I do not recall ever having this discussion with Dr testified he was worried about getting close to tho ureters but he was only there to assist in the operation I was concerned we were getting close to the ureters I said something to that effect I thought we must bo In dangerous territory said Dr Decision on appeal by midFebruary A decision on Ford appeal of his revoked privilege will likely be made within a month A spokesman for the Ontario Hospital Appeals Board said board members will have to draft a decision and bold a meeting to discus It and make any m nts Appeals Board can take up to three month to make Its decision but Board Chairman Tboma Wood finishes his three year term on tbe board In mid- February A decision can be expected before Mr Wood his term the spokesman ad The appeal board can grant restricts privileges to Ford at the hospital or it can grant lull privilege The other option open to the Board hospital Should the latter decision be made Ford a t r all had conducted a dye test earlier In the woman pregnancy help Iden theureters But Dr Trant said a method in the Insertion of catheters which give the anatomy a three- dimensional appearance to help assist In finding the ureters could be used Dr Ford said he have privileges to insert catheters and there was no Urologist In he hospital to perform the procedure In this particular case In hind He said Dr Ford initiated no special effort to identify the ureters before they were cut In another case Ford admit ted a patient referred to him by Dr to he hospital for a Caesarian section because the baby was in a breech position coming down the birth canal feet first Dr said he visited he woman In he hospital and he then told Dr Ford that he thought the baby was not in a breech position but was quite normal The woman had a Caesarian sec tion and the baby was found be In a normal position said Dr Trant There was no Indication at all for performing a Caesarian section He said he modified his referral procedures to Dr Ford hat In But Dr Trant testified about two other concerns he had about Fords practice In some cases he found Dr Ford used lata go and curettage ID and C operations for treatment of ramps and bleeding Involving Inter Uterine Devices Family physicians remove IUDs often It a very simple procedure to do It in the doctors office he said IUDs In another case Dr Ford properly performed a pap smear to find a cervical lesion which can lead lo cancer Trant I very happy about the treatment of that situation The Board then heard testimony from Dr Paul who together with his wife Debbie run an obstetrical practice in Georgetown Dr was the anaesthetist for Dr Ford In the delivery room for many of his deliveries He outlined a number of his con cerns with Ford practice for the hearing most of which repeated other doctors complaints about his practice Zenl said he sow a lot of necessary D and C operations on young women I didn llilnk the D and C operations were necessary surgical procedures I do that in my office I ve never had o take out an surgically He specifically cited a patient in Expert concerned with procedures An expert witness testifying for tbe Georgetown hospital Jan told the Ontario Hospital Appeals Board he would have many serious con cerns about Dr Jack Fords prac tice Dr Ernest Ewaschuck the Chief of Obstetrics and Gynecology at Oakville Trafalgar Memorial Hospital and an assistant professor at McMoster Medical Centre said he is concerned about how uptodate Dr Fords academic knowledge is in the field of obstetrics and He also listed a host of other pro blems with Fords practice in cluding his use of and tage D and operations for func tional uterine bleeding the ade quacy of his obstetrical care his treatment of premature ruptured membranes his care of a syndrome known as interuterine growth and he voiced concerns about Dr also said he is con about the high rate of mor bidity of Ford patients Dr Ewaschuck offered some com statistics between Ford practice the hospital and the Women College In Toronto In a 12month period beginning in April seven gynecologists at Oakville Trafalgar performed D and operations At Women Col lege 15 gynecologists performed D and C operations in a 12 period Dr Ford performed D and C operations in a IS months period beginning June 19B3 said Dr During those timo periods Trafalgar hospital per formed hysterectomies about per gynecologist Women College averaged just over hysterectomies per gynecologist during their 12 months period and during Dr Ford a 15 months period he performed 83 hysterectomies Ewaschuck In earlier testimony by the Georgetown hospital Director of Records Susan Merrick the Board heard that Fords use of mid forceps in delivering babies and his use of local anaesthetics as opposed to more intense freezing usually ad ministered by an anaesthetist Is significantly higher than other doc tors at the Georgetown hospital Ms Merrick said in 19J85 Dr Ford delivered eight per cent of his deliveries that year with the use of mid forceps Valerie Kumar the only other obstetrician at Georgetown hospital used mid forceps four per cent of the time that year Dr Drew Allin an expert witness for the hospital earlier testified that five per cent is about normal use of mid forceps Dr Ford used midforceps In eight per cent of the deliveries in 1885flfl and In per cent of the deliveries in B687 said Mb Merrick Dr Ford who had earlier testified that he prefers using local anaesthetic instead of more intense freezing commonly used called an Epidural Block or a Pudendal Block used local anaesthetic in per cent of the mid forceps deliveries while all other doctors In the hospital used a local In only 196 per cent of their mid forceps deliveries said Ms Merrick Other doctors used a Pudendal Block in per cent of their mid forceps deliveries and an Epidural Block In Just over per cent of the cases said Ms Merrick Dr Ford used them in 1 per cent and per cent of his midforceps deliveries respectively she Id Questioned by hospital lawyer Joshua Liswood Dr Ewaschuck said he studied of Dr Fords pa tlent charts with hospital Executive Director Mark Rochon and Medical Advisory Committee
