Once the patient is released from hospital, caregivers often discover gaps in the healthcare system, in terms of co-ordination of homecare services and lack of adequate support staff and services, according to members of the Townsend Smith Foundation, established to provide hospice services and ultimately, a hospice, in north Halton. On top of dealing with the imminent death of a loved one, the stress of maneuvering the system can be anguishing, agreed foundation committee members. "We need a very upgraded level of homecare. If we can't look to the healthcare system for what's necessary. It very much has to become a community issue," said Lyndon Smith, whose wife Tessie passed away in 2009. "As her (Tessie's) spouse, I felt I deserved time with her at the end but the system denied me that," said Smith. "You can't just sit down and be with them. You're providing medication and sustenance."
