"This is an exciting time for Halton Healthcare. The major infrastructure projects at our three hospitals are well in hand. Now we are embarking on a strategic planning process to chart the future pathway for our hospitals," said Denise Hardenne, President and CEO, Halton Healthcare. "We are reaching out to the people in the communities we serve in Halton Hills, Milton and Oakville and inviting them to join the conversation about Pathway 2020 and help us create our vision." When Halton Healthcare was established in 1998, the Mission and Vision were clear: revitalize and expand the hospitals to match the growth and aging needs of the communities of Oakville and Milton. This direction grew to include Halton Hills when Georgetown Hospital joined Halton Healthcare in 2006. "The completed Georgetown Hospital project is delivering amazing results. This spring, we will break ground on a significant expansion of Milton District Hospital. In December we will open the New Oakville Hospital," said Ms Hardenne. "We are developing strategies to leverage these investments in facilities and technology to improve the patient experience. We are reaching out to our patients and their families, our communities and our healthcare partners to hear what they have to say." Get involved as a Patient or Family Advisor Halton Healthcare needs your help ­ and your advice. We are partnering with patients and families at all levels of the organization to create a more patient and family-centred culture of care at our three hospitals which include Georgetown Hospital, Milton District Hospital and Oakville Trafalgar Memorial Hospital. Patients and their families have always played a key role in improving the patient experience at Halton Healthcare through patient satisfaction and website surveys, focus groups and advisory committees. Now we are looking for patients and family members or caregivers to get involved with Halton Healthcare in two new ways. You can participate in the future of your healthcare by applying to be one of our new Patient and Family Advisors or by joining the recently launched Patient and Family Advisory Council (PFAC). To qualify for these roles, you must be a patient and/or a family member or caregiver of a patient who has received care at any Halton Healthcare hospital within the last two years. Patient and Family Advisors Complete our online survey now! Please get involved in Pathway 2020 and share your vision. Be part of the conversation by completing our online survey. The input Halton Healthcare receives from this community survey will be used to inform our new Strategic Plan, which will be launched later this spring. The online survey will take approximately 5 minutes to complete and will be open until April 10, 2015. All responses are confidential. These advisors will be invited to sit on various committees throughout the organization, including working committees and teams, to provide the patient/family perspective on projects from development and implementation through to the evaluation stage. Patient and Family Advisory Council The Patient and Family Advisory Council will also provide a way for the hospital's staff to connect with patients and family members, working together to improve the patient experience. This Council, which requires a 2-4 hour commitment per month, will serve in an advisory capacity at the corporate level and make recommendations on the delivery of care and patient experience-related matters such as visiting hours, parking and accessibility.
