When asked if death from eating disorders is possible without taking a drug, Dr. Rao said yes. She dismissed the notion, however, that Young's death and the fact she was taking Cisapride was a coincidence. "It is more than coincidence. It is a factor," she said. "Yes, it did have a role." The company maintains that Cisapride is to be used only for gastro intestinal motility problems. Young died on March 19. 2000 after her heart stopped while she was at home. Family, neighbours and emer gency personnel managed to re-start her heart, but she was later pronounced dead at the M cM aster University Health Centre in Hamilton. Young, a Grade 10 student at Oakville-Trafalgar High School, had been taking the drug off and on since 1999. Janssen Pharmaceutica, of Titusville, N.J. announced in March 2000 that the drug would no longer be marketed in America after July 14, 2000 - except in special cases. Health Canada pulled Cisapride (known as Prepulsid in this country and Propulsid in the United States) from Canadian pharmacies August 7, 2000 due to the possibility of "rare, but serious, heart complications, including ... irregular heartbeat rhythms and sudden death." Dr. David Eden, Regional Coroner for Niagara, called the inquest and is presiding over the proceedings which could take weeks to finish. On Monday, Young family attorney Gary Will made issue of the bulletins issued by Cisapride's manufacturer, such as the one from September 1998 which warned of irregular heartbeats that could prove fatal. The Oakville pharmacist, Dan Harris, whose store dispensed Young Cisapride six times between January 1999 and March 2000. however, said the patient sheet issued when her first prescription was filled contained none of the warnings issued to American pharmacists six months prior. The need for more explicit warnings was underscored by Dr. Alan Brown, the child and adolescent psychiatrist Young saw twice before she died, who possessed an uncomfortably close con nection to the drug. "My wife was on (Cisapride)," said Dr. Brown. "I thought it was safe." Unfortunately, Dr. Brown told the inquest, he was unaware of the drug's side-effects and its links to a number of deaths. When he read a company warn ing on March 27, 2000, the first person he thought of was his wife. The second was Vanessa Young. He then took the warning across the hall to his wife's doctor who was likewise unaware of the drug's dangers. If he had that bulletin while he was treating Young, Dr. Brown said he would have called Dr. Ron Mittler, Young's family physician, and been more "aggressive" in questioning his patient about bulimia. Physicians are often "inundated" with mail, said Dr. Brown, and he sug gested that serious warnings need to be "highlighted" more appropriately to get their attention. Young was referred to Dr. Brown by Dr. M ittler for several "significant emotional and psychiatric issues" of which bulim ia was not the "main issue." At that time he understood that Young had been over-eating then vom iting since 1998, but was told by her that she'd stopped two weeks before her initial appointment on Jan. 5, 2000. Dr. Brown characterized Young, when they first met, as an "alert, co operative, attractive" 15-year old who did well in school and seemed well adjusted. Dr. Brown said his five-foot eleven, 137-pound patient just seemed "fixed" on weighing 135 pounds. Young also told him that Cisapride was prescribed by Dr. Mittler to quell the urge to vomit. "I had no concerns at that point in time," said Dr. Brown. "She was the picture o f health when she came in." Young and Dr. Brown met again on March 15. 2000 - four days before she died - at which time Young told him that her bulimia had been under control for 10 to 12 weeks. Dr. Brown later said that bulimics often hide the fact they binge and vomit and can appear quite healthy. At that point, however, Dr. Brown said he was still trying establish trust with Young. The environment ministry has not granted an environmental assessment for any of the 13 private electrical generating plants built in Ontario over the last decade. O ntario's Environm ental Assessment Act is soon to be replaced by O ntario's G uideline for the Environmental Screening Process for Electricity Projects and Sithe has fol lowed the new rules. "We remain open to discussing any concerns our neighbours may have," said Sithe vice-president Duane Cramer. Burton said he still feels "kindly" toward Sithe, which has been "open," but noted, "I think their definition of dealing with our concerns is talking about it with us." "If an activity causes a negative impact on public health, it shouldn't be approved," said Cramer. "No development activity is going to result in no impact." Ward I Councillor Kevin Flynn and Ward 3 Councillor Keith Bird repeatedly asked if Sithe was building the cleanest plant it could. Cramer responded, "We feel w e're doing what's appropriate for this site." Councillors heard that S ithe's Massachusetts and Washington plants appear to have lower emission levels, while two New York plants aren't near residential areas. Cram er said Southdown Plant would have lower particulate emis sions, which he called "the nasties" and least desirable. "People are worried about their kids and their lungs, it doesn't matter what you call it," said Flynn. Mary Trudelle, of JCRI. blasted O ntario's environm ental standards. "The tragedy in all of this is that we know they'll meet the (MOE) stan dards." "They (Sithe) should do at least as much at Southdown as they're doing in the U.S. If Sithe can accommodate Massachusetts, why can't they do it here?" said Trudelle. Potential low frequency noise problems in nearby homes could be solved with more costly construction techniques, so councillors added noise attenuation demands to their list.
