Recipients, awarded in two categories, included: · Creating Simplified and Equitable Access to Diabetes Care, led by Halton Healthcare and Trillium Health Partners; · Partnering to Improve Care for High Needs Clients with Acquired Brain Injury and Behavioural Challenges, led by Peel Halton Dufferin Acquired Brain Injury Services and Extendicare Halton Hills.
