- Lewis Erwin II, 40, died of heart failure Dec. 15 [1991] at Evanston Hospital. .
Mr. Erwin was born in 1951 in Warsaw, Ind.
He is survived by his wife, Jacquie; three children, Eleanor, June and Will; his father and mother, William and June Erwin of Bourbon, Ind.; a brother, James; and a sister, Hope. Services were held at Winnetka Congregational Church on Dec. 18.
