Establish new in-home support services Coordination Unit is the Nursing r 'The aging services community has long recognized the need to reduce the number of older persons unnecessarily receiving institutionally based services," commented Charles D. Johnson, executive director of Region Two Area Agency on Aging when announcing the Case Coordination Unit (CCU) designations in each of the eight counties within Region Two. The CCU agencies in each Region Two county have the resources to do a complete job in communicating and providing • the service coordination bet ween the in-home supportive services and the community based services to the elderly in their area. Here's how. Through the Case Coor dination Unit program, older persons and their families have a central intake point to learn about and gain access to ser vices and benefits. From these units, Outreach, Information • and Referral, Individual Assessments, and Case Management Services are explored. These services are available for all older persons, with special emphasis being placed on serving the frail, vulnerable, homebound elderly. Through the CCU, older persons and their families also have the individual's eligibility determined for services available through the Com munity Care Program. These include Homemaker, Chore- Housekeeping and Adult Day Care. At the heart of the Case Home Pre-Screening Program which went into effect in Illinois on July 15. Effective that date, anyone 80 or older (or under 60 and disabled) who planned to enter a nursing home and receive public assistance, must first be evaluated to see if in stitutionalization is their best option. If it is not, the CCU program is available to find supportive services which will keep them in their own homes for as long as possible. The Pre-Screening Program operates on a point system to evaluate the client's resources ~ how well he or she is able to function routinely: any, is required; what help, if any, is already being provided. The .biggest problem facing the Case Coordination Unit ogram is making certain those who need its services know it exists. "It is the responsibility of all advocates and professionals - hospital per sonnel, ministers, local service organizations, meal centers, social workers and all others in contact with the elderly - to spread the word," Johnson said. The Case Coordination Unit agencies in the Region Two Area include the McHenry County Senior Citizens Council with headquarters in Crystal Helen Carnaby of the AAcHenry County Senior Citizens Council accepts formal Case Coordination Unit designation from Region Two Area Agency on Aging at the Region Two 1983 annual meeting held in Lisle.
