OAKVILLE BEAVER Thursday, August 20, 2009 · 12 Health Oakville Beaver Halton's diabetes program recognized nationally Program began at OTMH in the 1960s and now helps residents living across Halton The Halton Diabetes Program at Halton Healthcare Services (HHS) has been granted a five-year Certificate of Recognition by the Canadian Diabetes Association (CDA). It is for meeting the Standards for Diabetes Education in Canada. HHS, which operates Oakville, Milton and Georgetown hospitals, has diabetes program centres in all three communities and partners with Joseph Brant Memorial Hospital in Burlington. The CDA review requires its member organizations to meet 17 standards of program effectiveness in ensuring patients have, and are using, the available resources to effectively self-manage their diabetes. It evaluates the awareness levels of the program in the community and its effectiveness in early identification of risk factors, prevention of complications and cost effectiveness. "We had to audit patient charts to determine how many diabetes patients are admitted to hospitals in the region and what hospital resources they used. We also surveyed our patients to determine both their knowledge of diabetes and their ability to selfmanage their condition," said Cathy Benbow Plewes, manager of the Halton Diabetes Program. "We had to demonstrate that our educational programs were clientcentred and that our education services, and both our human and physical resources, enable the achievement of the CDA Review Standards." "The CDA Standards Recognition Review is a voluntary program. We decided to participate in it to mark the Halton Diabetes Program's 10th anniversary in 2008/09," said Plewes. The Halton program has grown by leaps and bounds over the last decade -- moving from a pilot project in 1997 to a regional program with a staff of 30 across its four sites. "Even in very busy and challenging times, the Halton Diabetes Program has excelled in serving our communities," said HHS president and CEO John Oliver. "This amazing performance can be attributed to our Halton Diabetes Program's staff, physicians and volunteers and their endless efforts and dedication to providing quality patient care and education," said Oliver. The Certificate of Recognition will be presented to representatives of the program at the International Diabetes Federation Conference to be held in Montreal in October. The program serves adults with diabetes and their families. It receives funding from the Ministry of Health and Long-Term Care (MOHLTC), individual local hospitals, HHS and Joseph Brant Memorial Hospital. Through education and counselling, the staff hopes people with diabetes will gain information and skills required to self-manage diabetes. The roots of the program date back to the 1960s when it began as a diabetes educational program at the Oakville-Trafalgar Memorial Hospital (OTMH) -- with a dietitian one half-day per week. A few key individuals, most notably Dr. Grahame Owen and Jane Harper provided the commitment and leadership to expand diabetes services at OTMH. In 1997, with Oliver's support, the service was formalized as the Oakville Diabetes Centre, a oneyear, pilot project with funding for a full-time team of a nurse and a dietitian. Based on the success of this project, the regional program was launched in 1998. For the first seven years, the HDP had about seven full-time staff nurses and dietitian diabetes educators for all of Halton. In 2006, the MOHLTC provided more funding to double the staff to about 14 diabetes educator positions. Diabetes is a chronic condition in which the body either stops making insulin (as in Type 1 diabetes), or cannot properly use the insulin it makes (as in Type 2 diabetes). Insulin is a chemical made by the body, which helps the body to change glucose, obtained from food, into energy. Most people with diabetes have type 2, and are middle-aged or older.
