Elkalza is currently a regional patient and caregiver engagem ent coordinator at the M ississauga Halton LHIN. She initiated the Health Links Patient Partnership (HeLPP) ini tiative, w hich is a patient and caregiver group that w orks to em bed patient and caregiver voice into the health care system to foster im provem ents in the LHIN region' s approach to caring for pa tients. Care Coordination Program of W ork (CCPW ) initiative, from the M ississauga Halton LHIN, w on a 2 0 1 7 M inister' s Medal Honour Roll Award in the team category. The CCPW is a m ulti-dim en sional initiative created by the M ississauga Halton LHIN. The primary goal of the initia tive is to enhance the M ississauga Halton LHIN' s regional care co ordination role, and strengthen its relationships w ith health and social service providers, in clu d ing primary care to ultim ately im prove care for patients.
