15 · Thursday, September 27, 2012 OAKVILLE BEAVER · www.insideHALTON.com NEWS AND INFORMATION FROM OAKVILLE-TRAFALGAR MEMORIAL HOSPITAL SEPTEMBER 2012 New Oakville Hospital Update Work on Oakville's new hospital has been progressing steadily over the past year and thanks to a webcam, mounted high on one of the cranes, curious onlookers can get a bird's eye view of construction at the site. Constructing the physical building is just one part of the work that lies ahead. Another important section is preparing people for all the changes that the move to the new space will bring - a process commonly referred to as Operational Readiness. In order to get ready for these changes, Halton Healthcare Services (HHS) launched an Operational Readiness initiative which has been progressing steadily since early 2012. A lot of this work is being done `behind-the-scenes' as the organization works through the challenging task of ensuring that the move to the new facility happens smoothly. The ultimate goal of Operational Readiness is to ensure the organization can continue to deliver safe and effective care as its staff, physicians and volunteers move into a new environment, with new configurations, processes, technology and equipment. Over the next several years, HHS will work to develop a comprehensive move strategy that will ensure the smoothest transition possible for both patients and staff. Visit us on-line at www.newoakvillehospital.com to view the camera and find out more about this exciting project. Patient Safety Expo Please join us for Halton Healthcare Services' (HHS) 6th annual Patient Safety Expo. These one day Expos will showcase up to 60 quality-safety displays. The expos provide an opportunity for HHS staff to share and communicate patient safety initiatives with colleagues across the three HHS hospitals, raise awareness of patient safety concerns and solutions amongst physicians, staff, volunteers and the community, and celebrate our achievements. Join us for a fun day of learning and vote on your favourite display! Oakville-Trafalgar Memorial Hospital Monday, October 29, 2012 11:00 a.m. - 3:00 p.m., Auditorium Gearing Up for Accreditation In May 2010 Accreditation Canada awarded Halton Healthcare Services (HHS) an exceptional, three-year full accreditation with no conditions and recognized HHS for three of its leading practices. This remarkable accomplishment, which reflected the organization's unwavering commitment to quality and excellence, puts HHS in good stead as it enthusiastically gears up for another on-site evaluation by Accreditation Canada in May 2013. Based on national standards of excellence, Accreditation Canada provides healthcare organizations with an external, peer-review process to assess and improve the services they provide to their patients. Accreditation Canada uses Qmentum, an interactive, continuous process that operates on a three-year cycle; it starts with internal HHS accreditation team self-assessments and culminates in a four-day, on-site peer evaluation by an Accreditation Canada survey team. Clinical care, governance, leadership, medication management, and infection prevention and control are just some of the key system-wide areas that are assessed. Accreditation Canada's focus has changed over the years from reviewing policies and procedures to actually seeing them translated into action at the frontline by our caregivers. It looks at how we regularly review and continuously improve our care and processes throughout our entire organization. It also examines our organization's worklife, safety cultures and governance as well as our effectiveness in meeting the needs of our rapidly growing communities. Accreditation is about acknowledging the wealth of great quality and safety initiatives that is already occurring at HHS hospitals daily. The staff, physicians and volunteers here are committed to excellence. As an organization, we look forward to the opportunity to demonstrate our knowledge, explain our new initiatives, and showcase our best practices.
