Safer Elder Care Program The Safer Elder Care Program at Halton Healthcare Services (HHS) is an award winning initiative that recognizes the unique needs of the older adult population. It provides a culture of care that addresses common interrelated geriatric issues such as delirium prevention and management, safe mobility, falls prevention, skin care and the prevention of urinary tract infections. The HHS' Safer Elder Care Program has developed pamphlets to help educate patients and families on Safer Elder Care, Falls Prevention and Understanding Delirium. Thank You Servier Canada Servier Canada recently donated $15,000 to Halton Healthcare Services' Heart Function Clinic at Oakville-Trafalgar Memorial Hospital. This gift will be used to provide additional dedicated clerical support for the clinic, which is housed within the Cardiorespiratory Department. The Heart Function Clinic educates and monitors patients with heart failure after they are discharged from the Hospital. The clinic teaches patients and their families how to live with heart disease with the goal of maximizing the quality of their life.
