The HDP serves patients throughout the region of Halton in many community locations as well as these hospital locations: Oakville Trafalgar Memorial Hospital, Milton District Hospital, Georgetown Hospital and Joseph Brant Community Health Centre in Burlington. The Program provides care and support to people with diabetes and equips them to take charge of their health and well-being through a self-management approach. It provides individual appointments, group programs and workshops at a variety of locations in Halton Region and West Mississauga. The HDP also partners with other programs within the hospital setting such as: Dialysis Unit (Burlington and Oakville), Gestational Clinics, Oncology Department and inpatient areas including the Emergency Department. Your Health Osteoporosis awareness & new clinic November is Osteoporosis Awareness Month - did you know that one in three women and one in five men will break a bone due to osteoporosis? November 2016 Halton Healthcare opened a new Complex Osteoporosis Clinic in September 2016 at Oakville Trafalgar Memorial Hospital. The clinic provides a comprehensive assessment for elderly patients with osteoporosis or those at risk of developing osteoporosis in the future. The clinic is led by Dr. Aliya Khan and is supported with a nurse practitioner and a physiotherapist. Patients admitted to hospital with fractures or seen in the fracture clinic can be referred to the Complex Osteoporosis Clinic. Adding art to the healing process Never underestimate the healing power of art and how it can work wonders in lifting a patient's or visitor's spirit. "A piece may trigger a memory or stimulate an interesting conversation...It allows patients to go on an adventure (mentally), leaving their nursing unit and hopefully their health worries behind." The OTMH Art Council was established in 2015 to develop an art program at the new hospital featuring a variety of intriguing exhibits to inspire patients and their families. The collection includes art pieces from the legacy hospital as well as new donated or loaned pieces. The OTMH Art Council has recently received a Visual Arts Project grant of $8,500 from the Ontario Arts Council to curate the next art exhibition at OTMH called `Crafted' which focuses on the healing aspect of crafts. The exhibit, which will be displayed at OTMH for a year starting in December features art grounded in crafts such as knitting or other past-times. The Halton Diabetes Program team is here for you! Polar Bears sighting at OTMH They look so real, you might actually think that these majestic creatures are migrating to Oakville. The polar bear and cub sculpture, titled Raft of the Ursa, which was recently installed at the northeast corner of OTMH's property, was created by Bert Jackson, an Oakville visual artist. Warm, cozy and cute Special thanks to LeeLee Hats for generously donating 2,160 knitted baby hats to the Maternal Child Unit and Obstetrics Units at Halton Healthcare's three hospitals. LeeLee Hats is a non-profit group that works in partnership with Windsor Public Library Knitting/Looming Groups to provide babies in hospitals with handmade hats.
