Researchers have concluded that changes in the social environment and in individual health habits account for much of the decline. The shift is toward health promotion and disease prevention. At OTMH we, recognize that personal choices and habits play a big part in most forms of cardiovascular " disease, therefore we established a cardliac rehabilitaâ€" tion program to help offset this disease. As part of the recovery from a Heart Attack, a patient nurse * educator visits all patients on a one to one basis to discuss the disease process, indlividual risk factors and assist with making lifestyle changes. This involves activities to control smoking, blood pressure, diabetes, choâ€" ; lesterol dlisorders and environmental stress, as well as promoting an exercise routine approâ€" priate for the patient. This program includes input from all members of the patient care teams, ie: Dietician, staff nurse, pharmacist, social work, physiotherapist, etc. The education process involves the patient‘s family and friends in order for them to undlerstand the disease and their roles in the rehabilitation process. By taking the preventative approach, individuals become better eclucated and understand how they can modlify their habits into a healthier lifestyle. Our primary goal is to edlucate indlividuals to take better care of themselves and reduce the risk of heart dlisease.
