"On April 2 FPO McKibben observed wa- ter spray patterns on the ceiling consistent with a sidewall sprinkler in- stalled upside down. The sprinkler being installed upside down caused the water to be directed at and across the ceiling in- stead of the area down- ward and away from the wall," reads the report. "If the sprinkler had been installed correctly, it would have sprayed the water directly over the oc- cupant and surrounding area. It cannot be con- firmed if this would havefirmed if this would havef prevented the death of the individual, but it should be noted that this could have had a different out- come." The report said the in- vestigation also found is- sues with the building's fire safety plan.fire safety plan.f Under this plan, the re- port said, either property management or the su- perintendent is responsi- ble for moving the resi- dents who are unable to evacuate their unit. The report said discus- sions with the property manager revealed they were aware of the plan but said it could not be exe- cuted because the eleva- tors are not to be used during a fire and it is not possible to get the wheel- chair-bound residents down the stairs. "She said that they were required to stay in place with the doors closed, while the rest of the staff and abled per- sons evacuate the build- ing," reads the report. "Chief Fire Prevention Officer (Gary) Lafram- boise and FPO McKibben also reviewed the 'March of Dimes Canada's Train- ing Scenarios for Oak- ville Supportive Hous- ing', which describes the responsibilities of the care staff during an emer- gency. "In short, the staff are to close the doors to the occupant units if safe to do so and leave the build- ing. "All residents are to 'Stay in Place' regardless of the fact that the resi- dents cannot physically perform the tasks neces- sary to protect them- selves in place. An exam- ple of this would be taping cracks around the door to limit smoke migration in- to their unit." Calls to Precision Property Management were not returned. Jason Lye, vice-presi- dent of the March of Dimes Canada Non-Profit Housing Corp. communi- ty support services, told the Oakville Beaver the organization rigorously adheres to all safety pro- tocols required in the op- eration of an apartment building. He said this includes completing regular safe- ty inspections and con- ducting bi-annual fire drills. "We also retain the ser- vices of a professional property management company to ensure that all applicable standards are met, including hous- ing standards, the fire code and safety proto- cols," said Lye. "Tragically, despite these efforts and the work of property management staff, a longtime tenant lost her life. Although it would not be appropriate to comment at this time, specifically upon the cir- cumstances of this tragic death, in view of ongoing legal proceedings, we have always been commit- ted to and continue to be committed to providing affordable housing, which meets all the ap- propriate safety stan- dards, which welcomes all people and, in particu- lar, is accessible to per- sons who live with disa- bilities." Following the blaze, the report said, the fire department issued an in- spection order to ensure all other sprinkler heads within the 259 Robinson St. building were in- stalled correctly. The report also said the property owner was directed to develop, sub- mit for approval and im- plement a fire safety plan, which includes evacua- tion procedures for all residents. The matter is sched- uled to come before a pro- vincial offences court in Burlington, on Sept. 13.
