Notes: Joseph R. Choklad, 65, of Henderson, Nev., and formerly of Lake in the Hills, died Wednesday, Dec. 15, 2004, in Henderson. Survivors include his wife, Carol; four children, Bret Choklad, Scott (Charlene) Choklad, Patty Choklad and William Choklad; his mother, Hilda Choklad; a brother, Steven Choklad; and a sister, Reberta (Dennis) Sundberg. He was preceded in death by his father, Harry Choklad. The visitation will be held at Willow Funeral Home, Algonquin.
