- Mounted set of five medals consisting of:
- a Military Medal (M.M) bearing the bust of King George V on the obverse and the wording "FOR/BRAVERY/IN/THE/FIELD" on the reverse, suspended from a ribbon of blue, white and red. The recipient's information is impressed on the rim "602366 SJT. J.W. CHIVERS 14-CAN. INF."
- a bronze Inter-Allied Victory Medal impressed as above on the rim.
- a silver Canadian Voluntary Service Medal
- a silver WW II War Medal
All five medals are mounted on a thin bar of wood
