On November 28th, a pair of motivational speakers from London, Ontario came to speak to the students. Apparent Leg Length Discrepancy Question: Telma I have been told that one of my legs is shorter than the other and it was suggested that I put a heel lift under my shorter leg to even them out. What do you think? In the seven years I have practiced as a physiotherapist, there have only been a handful of times when 1 have seen a client client with one leg actually longer than another. On the other hand, clients often present with one leg apparently longer Ilian the other. This Is because one hip is higher than the other. For example, a patient came In to see me for her low back pain. She had been told that her right leg was shorter than the left and a heel lift was put Into her orthotics on the right side. When I examined her the leg length was line, but the right hip, the same hip where she had the heel lift, was higher than her left hip. She had an apparent leg longth discrepancy, discrepancy, not an actual one. A patient like this needs to have the muscles supporting the right hip stretched so the hip can move down freely to make the two hips oven. This will help with the back pain because when the hips are oil killer, It will put the back muscles muscles Into spasm. To make a long story short, a heel lilt will help only if that leg Is really shorter, but it may hurl if the leg is only apparently shorter. A leg may appear shorter when muscle spasms of the back and hip muscles pull up on the hip of the same side. The muscle that spasms In the front Is usually the psoas muscle, muscle, and the one that spasms in the back Is usually the quadra- tus lumborum. When considering the need for a heel lift, one needs to determine whether there Is an actual leg length discrepancy or just an apparent one. In the vast majority cases I have seen In clinic, it's a case of one hip being higher than the other. In this case a heel lilt will not help the problem and It may actually make It worse. These clients usually come In with low back pain. In treatmenl of these clients It Is essential that the muscles that are pulling the hip up be properly realigned. This Is best accomplished through a course ol physiotherapy, not with a heel lilt. In cases like this, tho best place 1er the heel lilt is In tho clients'pockets.
