Worth, Ph.D. in Psychology from Boston U., post-doctorate training in Family Therapy from Menninger Foundation in Topeka KS; family therapist in Oak Park IL; in 1968 co-founded Family Institute of Chicago at Northwestern U., later co-founded Phoenix Institute of Chicago, after 5 years returned to Family Institute, retired in 1999 to serve as consultant to mental health clinics; preceded in death by his brother, David Butler and sister, Barbara Hibbard
