L a r n h , M . D . , N e w s A m e r i c a S e r v i c e Problems with low blood sugar DEAR DR. LAMB: I have read that hypoglycemia is a symptom of other disorders rather than a condition in itself. But my doctor isri't checking for anything else. I was diagnosed as hav ing hypoglycemia over a year ago. I am on a sugar-free, high protein diet to control the symptoms (headaches, leg pains, weakness, tiredness and anxie ty). I am on no medication for the con dition. I do not understand how one two- hour glucose tolerance test could be high, up to 200 with 2-plus urine, and another low, under 50. DEAR READER: The real test of whether you have hypoglycemia or not is if you have a low blood glucose level at the same time you have symptoms. If you have symptoms and your blood glucose is normal, or if you have a low blood glucose level and do not have symptoms, you cannot make a diagnosis of hypoglycemia. The usual response to glucose loading is a sharp rise in the blood glucose level, and then a return to near normal within two hours. I would con sider it unusual to have a normal fasting blood glucose level and have a low level below 50 at two hours. That would be a good reason to repeat the itest. If your fasting blood glucose level is low and associated with symptoms, it says a lot about the cause of hypoglycemia and deserves further study to see if you have an underlying medical problem that is causing low levels. In reactive hypoglycemia it is com mon to have a high blood glucose level, followed by an abnormally low level, which causes symptoms. This form of hypoglycemia is always triggered by eating something that causes a sharp rise in blood glucose, even when the fasting glucose level is normal. It is a bit unusual to just study a two- hour glucose tolerance test for hypoglycemia, unless the patient develops symptoms and a low level that early in the test. The usual test is a five-hour test. You will understand the different forms of hypoglycemia better after reading The Health Letter 18-8, "Hypoglycemia, The Low Blood Sugar Problem," which I am sending you. Incidentally, many normal people have very low blood glucose levels and no symptoms. Values below 35 have CROSSWORD been observed in healthy people without hypoglycemia. DEAR DR. LAMB: I am writing to ask about a real problem, alcohol. When drinking the stuff, can it affect a male so that he cannot father children? ' Can it lower his sperm count so if you get pregnant you will only carry the baby about three months and have a miscarriage? I've been checked by my gynecologist and my body is fine. It's my husband I'm worried about. DEAR READER: Alcohol will decrease a man's sexual performance, but to my knowledge there are no studies that show that a man's use of alcohol will result in a miscarriage of a pregnancy. It could decrease the sperm count, as alcohol does decrease the level of testosterone in the body. Occasionally, a reasonable amount of alcohol may calm an anxious male and give him the courage to improve his performance. But the real effect of alcohol was well stated in Shakespeare's Macbeth, wherein Porter says, "Lechery, sir, it pro vokes, and uprovokes; it provokes the desire, but it takes away the per formance ..." Some men "think" their performance is improved but often they are like the drunk who thinks his driving has improved after drinking. Three no-trump would have no play with the lead of the spade king. There's a simple philosophy to the play of today's hand -- when you need a finesse to make your contract, assume it will work. Then figure out the probable location of the other key Declarer won the spade ace. Because he needed the club finesse to work, he placed the club king with West. But if West held the club king as well as the spade K-Q, how could he also hold the diamond ace? (As dealer he surely would have opened the bidding.) Concluding that East had the diamond ace, South played a heart back to his ace and led a dia mond to dummy, inserting the 10. When that card forced the ace, he was in good shape but still had to play with caution. East led a spade through South's jack to West's Q-10, and West played another spade. Declarer ruffed carefully with dummy's club jack, trumped a dia mond to his hand, and led the club 10. When that won, he was able to take another club finesse and make 11 tricks. What if West plays the queen of hearts instead of the third spade? Declarer must then resist the inclina tion to discard both his losing hearts on the K-Q of diamonds. Instead ha must trump a iowdi hand and then proceed finesse.
