"A Family Tradition for 132 Years" PORT PERRY STAR - Tuesday, March 31, 1998 - 9 Local antibiotics study moves into next phase By Chris Hall Port Perry Star Port Perry physician John Stewart has begun the next phase of his pilot project on the appropriate use of antibi- otics. After a year of conducting research in Port Perry on the prescription of antibi- otics, Dr. Stewart has received the okay from the Ministry of Health and 20 family physicians to continue his study in east Toronto. In PAACT 2 -- the second stage of Dr. Stewart's Pilot for Appropriate Anti- infective Community Therapy -- up to 20 family doctors at Toronto East General Hospital have agreed to take part in the nationally-recognized program. PAACT is a wide-ranging project aimed at identifying the proper usage of antibiotics. It's being undertaken in response to concerns that bacteria can become resistant to drugs if antibiotics are used in excess. "It's a series of case studies, specifically on community infections seen in general prac- tice," Dr. Stewart said of the project he has been working on for close to four years now. One purpose of the HS project 1s to inform John Stewart patients and doctors that antibiotics can only fight bacteria; they are useless against colds, the flu, influenza, ear infections and most sore throats. Physicians can come under pressure from patients to provide a quick fix for a virus, and pre- scribe antibiotics inappropriately. While word of PAACT has spread -- he is putting finishing touches on an article he plans to submit for publication in a major Canadian medical journal -- Dr. Stewart has also made his work known by way of electronic and print media. The initiative has been featured recently on CBC Radio, and in Toronto daily papers. "Some of the local doctors there are writing columns in their area papers to let people know what is happening and Discovery TV, which 1s doing a piece on bacterial resistance, videoed a bit on infections in the Toronto hospital, and that should air near the end of April," said Dr. Stewart. "I've been getting a lot of attention recently from everyone because of the stage we've moved to. This isn't a small- town survey anymore." Dr. Stewart hopes some day to see findings and recommendations from the PAACT project implemented nation- wide. The decision to move the project to Toronto was made after a year-long study in Port Perry. Research here showed that while the number of visits to doctors was steady, patients remained healthy even though the amount of antibiotics prescribed was reduced, said Dr. Stewart. "We're very pleased with the results from Toronto and from Port Perry," he sald 1n reference to the Oct. 1, 1996 to March 31, 1997 test conducted amongst local physicians. "We decreased the use of antibiotics here and the number of visits didn't increase; when we did use (the drugs), we leaned toward first-line antibiotics. During that same time the use increased across Ontario and Canada." But, the most important part of the pilot, he stresses, is the potential benefits for patients. "The entire focus is on the patient and how to provide better health care; we want to give them optimum treat- ment," said Dr. Stewart. "All the stake- holders -- patients, physicians and phar- macy companies -- are working together on this."
