(NOTARY FUILIC) (FEE 25 CENTS) (NOTARY'S ADDRESS) STATE OF ILLINOIS PAUL POWEU SECRETARY OF STATE This is the new combination notice thirty days-in advance Qf the approaching expiration date of driver licenses and application form for renewal of the license. The program initiated by Secretary of State Paul Powell to alert drivers to the need to renew their licenses will get under way early in J uly. The program to notify drivers thirty days in advance of the approaching expiration date of their driver licenses will get under way early in July, Secretary of State Paul Powell said this week. The new law directing the Secretary of State to issue these notices becomes effective July 1. The first drivers to be notified will be those whose licenses are due to expire on Aug. 1, Mr. Powell said. that, for the program to be successful, it is imperative that drivers observe the requirement of the law that they notify within ten days of any change in name or address. He emphasized that failure to receive the notice does not relieve the driver of the requirement to have a valid license at all tiems. Mr. Powell also reminded drivers that, in conjunction Secretary Powell pointed out with the notification program, when they apply for renewal of their licenses "after the expiration dale of the old license, the new license will be valid for three years less the amount of time the old license had been expired. The old practice of issuing renewal licenses valid for three years plus the time remaining until the applicant's next birthday, in cases where the old license had expired, is being discontinued. The notice is in the form of ! an application blank on which the name, address and physical description of the driver are already printed. Immediately below this area, is space to enter any corrections, but only corrections are to be entered there. In the lower part of the form are nine questions which the applicant must answer pertaining to his vision, physical condition,, whether the license has ever been revoked, suspended or restricted, or is being held in lieu of bail, whether the applicant has had any traffic law violation cpnvictions in the last twelve months , h o w man y m i 1 e $ driven in the last twelve months, and whether the applicant has ever been married. The question concerning marriage is directed primarily to minors, who, if they answer "no," must accompany the application with the written cpn* sent of the parent or guardian. Raising from 18 to 21 years the period during which a|n applicant must have parental or guardian consent to obtain a license was another nart of Secretary, Powell's legislative program enacted in 1965. At the bottom of the blank are spaces for the applicant's signature and the seal of a notary public.
