DON PEASLEY PHOTO A remote control x-ray television system - one of the few in the greater Chicago area - is called "the greatest tool a radiologist can have" by Dr. Nicholas I. Gemell. Dr. Gemell operates the $50,- 000 machine at McHenry hospital, where it has been in use for several months. This machine occupies two rooms. In one is a fluoroscope which makes x-ray pictures of a patient on a tilt table. A special TV camera views the fluoroscope screen and transmits the picture to the other room, a control room where all the equipment can be operated by remote control while the patient is seen through a large lead-shielded window and his x-ray is seen on the TV camera. "The increased workload at the hospital has created some problems in trying to accomplish everything in a limited number of working hours," said Dr. Gemell. The machine he said, has doubled and perhaps even tripled his efficiency. Using this equipment, Dr. Gemell fluoroscopes at least fifty patients a week, many more than could be examined without it. To use an x-ray fluoroscope without television, Dr. Gemell explained, the radiologist must wear red glasses and stand in a darkened room 20 minutes to condition his eyes before operating the machine. He also must wear a heavy 12-pound special lead-lined apron to guard against radiation. The new system uses the TV camera to photograph the fluoroscope screen image instead of viewing it directly. It amplifies the image electronically and reproduces it on the TV screen and can be seen in broad daylight, the same as in a home. "The biggest advantage is that I can make an instant diagnosis without having to wait for eye conditioning," Dr. Gemell said. "I can switch from one examination to another without the 20-minute time loss, important in an emergency. Radiation is also another consideration, The small amount experienced by the patient is not dangerous. But the radiologist can get this dose several times every day in use of an rdinarjr fluoroscope. ""Another advantage of the TV circuit is that it can boost brightness of the picture several thousand times and the radiation is kept at a minimum. Adding a video-tape recorder in the future will allow the doctor to record what is seen 1 on the TV screen and file the tapes with the patients history for future use. He will be able to replay the tape at any time and run it slow motion for viewing such things as swallowing, which normally takes a split second, too quick for the eye. About five machines of this type are in use in the greater Chicago area and approximately seventy such units are in operation in all the United States and Canada. A radiologist, * an important specialist in the complex field of x-ray, is a licensed doctor of medicine specializing only in the use of x-ray in the diagnosis and treatment of disease. Dr. Gemell, a native of Kusnas, Lithuania, graduated from the University of Munich Medical School, Germany, wherehe received his M.D. degree. He took his residency training in radiology at the Ohio State University hospital in Columbia, Ohio, where he also served as an instructor at the Medical school. Before coming to McHenry in 1964, Dr. Gemell was for six years assistant director of the diagnostic X-ray department at Michael Reese hospital and Medical center, Chicago.
