The forms were mailed to homeowners who applied in any previous year for property tax relief, according to Allphin, whose agency administers the three-year-old circuit breaker program The distribution, he said, was timed to coincide with county mailings of property tax bills since information from the final property tax bill must be included in the tax relief ap­ plication. In Cook and Lake counties, final property tax bills are expected later this summer. Allphin pointed out that the single application form this year served two tax relief programs. One provides up to $500 to offset property taxes and the other, signed into law this year, provides from $50 to $100, depending on the ap­ plicant's income, in additional tax relief. The director said persons applying for the additional tax relief grant only should write "Additional Grant Only" across the top of the application form. Anyone eligible for property tax relief is eligible automatically for the ad­ ditional tax relief..And senior and disabled citizens living in residences not subject to the property tax, though not eligible for the property tax relief, can qualify for the new tax relief grant. Application blanks can be obtained on request from the Circuit Breaker Section, Department of Revenue, Box 4017, Springfield, 111 62708.
