LAMB: Much to my sur prise, I found out a month ago that I had cardiovascular disease after tak ing a stress test, and had to undergo surgery. I had a triple bypass and thank God I'm recuperating nicely. I have been put on a low-saturated- fat, low-cholesterol diet with a limit of two grams of sodium. I find it very dif ficult staying with my diet, especially at lunch. I'm not allowed delicatessen meat or fish, frankfurters or cold cuts. Getting a sandwich with these restric tions is almost impossible. I can only have desserts if they are made with skim milk. I've been unable to find pies, cakes or even cookies made without eggs and with skim milk. I would like to comply with my diet. I gave up my cigars. Can you simplify my problem? Also, I gave up salt and butter. Is there a salt substitute that is not harmful? DEAR READER: Those who do follow a preventive program with diet and exercise, as indicated under super vision, after a coronary bypass opera tion have the best outlook for the future. I sincerely hope you will be able to follow a low-fat, low-cholesterol diet. Note that you need to restrict all your fat, not just saturated fat. And you need to restrict your calories to the odium point that you are able to eliminate any excess body fat you may J?*^ev Most cardiologists wou"| that severely restricting intake was so important if also have high blood pressu gestive heart failure. That woulc your choices for a good diet easier. Fish could even be beneficial. What you can do will need to be adjusted ac cording to your response, in terms of your cholesterol - HDL-cholesterol (good cholesterol) -- and other fin dings. I think you can simplify your diet and meet your goals. I'm sending you my recent Special Report 48 of The Health Letter, "Preventing Heart At tacks and Strokes by Diet," to help you. Anytime you have to eat out you may have difficulty in getting low-fat, low- cholesterol items. You may need to pack a lunch. If sodium restriction Is not essential you could use low-fat cot tage cheese with fruit. Yes, you can use the salt substitute "No Salt." Desserts are a problem, but if you need to limit calories, you should do without them. Avoiding all tobacco also is im portant. DEAR DR. LAMB: 1 am enclosing a newspaper clipping stating that three men who received permanent artificial hearts were all heavy smokers for years prior to their heart surgery. A noted doctor comments in the article that he did not believe this excessive smoking contributed to their near-fatal Impairments. Does this sound as in credible to you as It does to me? Surely all those cigarettes couldn't have done them much good. I would appreciate your comments. DEAR READER: Sometimes what you read in print isn't exactly what a person said. There certainly is an abundant amount of evidence that cigarette smoking is a major factor in causing fatty cholesterol deposits In arteries that lead to heart attacks and strokes. And heavy cigarette smoking will Increase a person's chances of having a heart attack three times what It would be in a nonsmoker. To quote the latest edition of Cecil's Textbook of Medicine, 1985, "The in creased Incidence of atherosclerosis (fatty-cholesterol deposits) found in smokers at autopsy correlates with the degree of previous smoking activity" and "these changes are related In degree to numbers of cigarettes smok ed, and cigar and pipe smokers have impressive increases over nonsmokers as well." If de clarer did not at that time have enough tricks established for his con tract, down he would go Maybe the club suit would behave, with the K- small on side It was more likely that the club layout would be unfavorable If he gave up a low club immediately, he would have only eight tricks Finally, Katz led a low spade from his hand If West rose with the ace and cleared hearts right away, declarer could play a low club from dummy With the king, appearing, all would be well. When West actually played low on the spade, declarer won the queen in dummy arid played the jack of clubs Up came the king, but this was allowed to hold ihe trick.
