More and more individuals in their late 30s and 40s are turning to plas tic surgeons to help them win the war against middle age. "Baby-boomers readily accept aesthetic plastic surgery as a way of looking as young as they feel," says Dr. Otto Weiglein, Certified Plastic Surgeon at the Centre for Personal Surgery in Burlington. The most popular area of plastic surgery for this age group is liposuction -- for both men and women! Liposuction is the surgical vacuuming of fat from deep beneath the surface of the skin. The pro cedure is best suited for patients of average weight with excess fat in localized areas and good overlying skin elasticity. The tumescent technique involves injection beneath the skin of large volumes of salt water containing lidocain, a local anesthetic, and small amounts of adrenaline, a natural ly occurring hormone that shrinks blood vessels. By inject ing this solution to the fat prior to performing liposuction, the plastic surgeon numbs the tis sues and shrinks the blood ves sels. This eliminates pain and reduces and minimizes bleeding, bruising and swelling. Although some physicians perform tumescent liposuction under local anesthesia with sedation, Dr. Weiglein prefers to perform the surgery under gen eral anesthetic. "It's safer for the patient, takes significantly less time, and carries less risk of car diac implications to have a licensed anesthetist control the sedation." Another method which com pliments. traditional liposuction is ultrasound-assisted lipoplasty. The fat is first fractionated by the application of ultrasonic energy. The fractionated fat mixes with the body fluid and the solution infused by the sur geon. The fractionated fat is then removed using low-volume suc tion through tiny incisions. This results in less trauma to tissues, therefore, less discomfort and a shorter period of convalescence for the patient. Certain anatomic regions, such as the hips, poste rior back and central body regions respond preferentially to ultrasound-assisted lipoplasty. Patients need to be aware, however, that liposuction is no substitute for exercise and main taining a low-fat diet. But diet and exercise aren't always enough.
