- Circular silver disc with attached straight silver bar suspender through which passes a silk ribbon with gold centre, flanked by think stripes of white, black and blue.
The obverse of the medal bears the bust of George V while the reverse depicts a naked man on a horse trampling a shield and skull and cross bones with the dates "1914 1918". On the rim is impressed "189888 H.H.H. HOLMES 1 CAN INF"
