Toronto General Hospital is one large teaching hospital which has found that the intensive care nurse they are looking for just doesn‘t arrive at the bedside of their critically ill patients as a fully experienced clinical expert. There are, as yet, Often with such seriously ill patients, there may be combinations of cardiac, pulmonary _ and _ renal problems, all of which are turned over to the expert attention of an individual medical consultant in each particular field. Among these numerous experts, there is one person on the intensive care team who, because of her constant presence, may be able to give more information about the total, overall condition of that particular patient to medical specialists who see the patient only _ inâ€" termittently. That person is the intensive care nurse. Who is she? Where is she found? How is she trained? From a rural area, a farmer arrives with a erushed chest from a tractor accident and is rushed to the Respiratory Failure Unit. From a highway smashâ€"up, another patient goes imâ€" mediately to Neurosurgery. A surgeon, in a community which has no openâ€"heart facilities, sends a patient down for a heart valve replacement; and another refers back a patient who has previously had a pacemaker inserted into his heart and once again requires _ constant monitoring and adjustment in the Coronary Care Unit. The ambulance which every day swings into the emergency bay of Toronto General Hospital can be coming from almost any area in Ontario, including this one, where a family doctor â€" intent on giving his patient one more chance for life â€" has referred the patient to the Hospital and one of its eight, highly sophisticated intensive care 2847 DUNDAS ST. Similar program patterns are being adopted for official educational courses to be offered for the first time this Spring, endorsed by the R.N.A.0..Â¥ the Ontario Medical Association and the Ontario _ Hospital Association. ‘"The finest intensive care nurse is your very best, basic Devising such special courses is an established pattern for the Nursing Service Department of the Hospital. When Operating Room Technicians were first needed, the Hospital was the first institution to set up a program to train this new kind of paraâ€"medical perâ€" sonnel. When it was noted that nursing refresher courses might lure more retired nurses back to active duty, the Hospital designed such a program. The idea has since been taken over on a provincial basis by the Registered Nurses Association of Ontario. One _ hundred such specialist nurses are needed at a referral hospital like the Toronto General, in order to give roundâ€"theâ€"clock care in the Hospital‘s eight intensive care units. These specialize, among other things, in heart, lung and kidney diseases, as well as units for major surgery including neurosurgery. Because the need is there and because enough professional nurses with additional intensive care training are not, the Hospital has recently set up a series of Intensive Care Nursing Courses under its own Staff Development Program. few postâ€"diploma courses available for those who want to obtain specialized eduâ€" cation and training in this field where the nurse must know how to give care to an acute patient, use and care for such equipment as respirators, chest drains and various types of monitors. That‘s why we look for maturity, inâ€" telligence, stability, and initiative when we interview nurses seeking positions in our intensive care units, or asking for transfers from general wards." "This is a oneâ€"toâ€"one, or at most, a oneâ€"toâ€"two nursing function. It‘s very interâ€" personal and it takes a special kind of nurse who is comfortable being involved in an intensive relationship, providing constant care to only one or two patients at a time.
