Durham Regioniai Police reported a total gof 32 accidents for the period from 8 a.m. Sun- da to 8 a.m. Monday. Twenty-six accidents involved property damnage, two were per- sonal injury, and four were fail- ing to remain. Wallets stolen at shoe store Two employees in the staff washroom of a local shoe store had their wallets stolen from their purses last Thursday. The thefts occurred at the Shoe Terminal, located at 1615 Dundas St. E., in Whitby. One employee discovered about $45 missin g from b er purse after leaving the washroom. She advised another employee, who was in the washroom at the same time, ta checkc the contents of her purse. She discovered cre- dit cards missing but no cash Was taken. There are no suspects at this time and police are investigating. Car stolen from station An automobile was stolen from the Whiltby GO station last Thursday.' David Sait of Bentley Ct., Whitby, parked at the Brock St. S. station at .8 a.m. and then caught the train into Toronto where he works. Pruneau offers patients the assistance they need to plan for their care once thoy are discharged from the hospital. *Discharge planning is everone's responsibility,w says Primeau, and Whitby General Hospital puts that philosophy into practice. Their approach to disoharge planning is muftidisciplinary. nW. want to ,b. sure we've met the patient's physical, social, and emotional needsw Primeau notes. A team made Up of the senior team leader (head nurse), chaplain, dietitian, occupational therapist, physiotherapist, and discharge plan ner, does weekly patient reviews to assess needs and ider~ ify those people who may require, intervention from f he*discharge planner. This team approach Ieads ta more family conferences and a more blendedl approach to services. After visiting with a patient, Primeau may then contact the' family to find out what services they already have in place., Together they draw Up a plan to determine what MI happen once the patient lea ves' the hospital. This can involve' anything from helping a patient arrange for convalescent care, to making sure the patient has the equipment h. or she needs to function at home. Every effort is made to help a patient return and stay at home, but in some cases that just isn't possible. "if a patient is leaving the hospital and going to a nursing or retirement home, 1 work with them to make sure they have ail the necessary documentation," Primeau explains. wWe want to be sure that the appropriate level of care wîll be provided,% she adds. Primeau must stay on top of a wide range of community and social services in order to offer patients the information they need. For instance, a woman who has been beaten requires information on spécial services avallable to battered women and Pruneau needs to know where to refer her. The family of a patient with Alzheimer's Disease is looking for a support group -- Primeau can provide that information. Primeau is actively involved on the boards of a number of social service agencies. In addition, she does a fair bit of advocating on behaif of patients and their families for services not currently available. Primeau's is a department of one, so she has her hands full.
